AGREEMENT OF RELEASE AND WAIVER OF LIABILITY
I hereby agree to the following:
I understand the offerings of this workshop hosted by Jennifer Merritts are not a substitute for medical attention, examination, diagnosis, or treatment.
If at any time during the activities I feel discomfort or pain, I will inform Jennifer. Paramedics and my emergency contact will be called.
I may rest at any time during the workshop. I will listen to my body and respect its limits.
I represent and warrant that I am physically fit and I have no medical condition that would prevent my participation regarding meditating, laying down, walking, or sitting.
I recognize that it is my responsibility to notify Jennifer of any assistance or accommodations that I may need before 10/26/2024.
I accept that neither the instructors, nor the hosting facility, are liable for any injury or damages to my self or my property while participating in this workshop.
I understand that all payments are due by October 16th, 2024. Cancellations made by October 16th, 2024 are eligible for a 100% refund. Cancellations made after this date are not eligible for a refund. Refund rules are in affect due to associated costs such as the retreat site and preparations for this event. If you wish to transfer your paid spot to someone else please contact Jennifer Merritts.
Photography: I understand there will be photographs taken during some portions of this workshop for the purpose of future marketing material.
If I do not wish to be a part of such photographs, I will notify Jennifer prior to October 26, 2024.
Exclusion of Participation: In the unfortunate event a participant causes disruptions that endanger the continuity and quality of the workshop and/or the health of other participants, facilitators, or property the guest can be excluded from the participation of the workshop after an initial warning. If the participant is excluded, already paid costs are non-refundable and incurring costs due to any damage are to be paid by the guest.
I have read the above release and waiver of liability and fully understand it’s contents as well as the Refund/Cancellation Policies. I voluntarily agree to the terms and conditions stated above.
